Qualifications

• Bachelor's degree in social work required.
• Active Indiana LBSW/LSW/LCSW licensure required or feasible under case-manager role.
• Nonlicensed candidates may be hired only as Case Manager for nonclinical duties.

Responsibilities

• Complete social, environmental, family-safety, and recovery assessments using approved tools.
• Develop and update service plans with goals, due dates, and follow-up steps.
• Connect participants to housing, food, transportation, benefits, legal aid, and community resources.
• Provide outreach for missed visits, hospital discharges, and treatment interruptions.
• Coordinate with clinical staff, DCS, hospitals, and local health departments with valid consent.
