Job summary

Family & Children’s Services is seeking a Behavioral Health Case Manager II to support clients and families in coordinating services, resources, and advocacy to promote independent living and recovery. You will work within a trauma-informed framework, documenting progress and maintaining strong interprofessional collaboration.

Responsibilities include assessing client needs, developing recovery plans, ensuring confidentiality, and navigating Medicaid/Health Home enrollment where applicable.

Qualifications

• Must meet ODMHSAS and HOPE’s requirements for Behavioral Health Case Manager II certification.
• Case management experience in behavioral health or social services preferred.

Responsibilities

• Coordinate the needs of clients and families with available resources and advocate as needed.
• Assist clients in accessing community resources to maintain independent functioning.
• Develop individualized recovery plans focusing on goals, skills, and community integration.
• Maintain confidentiality and adhere to agency policies, CARF, and DMHSAS standards.
