Job summary

Valleywise Health is seeking a Behavioral Health Team Specialist to provide hands-on case management, rehabilitation, support, and crisis intervention for adults with serious mental illness in clinic and community settings. Qualifications

• Bachelor's degree in Behavioral Health or related field.
• One year of full-time behavioral health work experience with adults.
• Two years of full-time behavioral health work experience with an associate's degree.
• 18 credit hours in behavioral health and two years of experience.
• Must have a valid Arizona driver’s license and vehicle.
• Fingerprint card within 7 days of employment.

Responsibilities

• Provide case management, rehabilitation, support, and crisis intervention services to members and families.
• Meet members where they are, including in the clinic, their homes, places of employment, and throughout the community.
• Build supportive relationships with adults living with serious mental illness.
• Collaborate with members and the care team to develop and implement ISPs based on needs and recovery goals.
• Coordinate behavioral health services and community resources identified in each member's service plan.
• Help members navigate available programs, benefits, entitlements, and community resources.
• Provide services using evidence‑based, trauma‑informed, and recovery‑oriented approaches.
• Collaborate with members, families, clinicians, and other care team members.
• Respond to behavioral health needs and crisis situations.
• Maintain HIPAA-compliant confidential information.
