Qualifications

Responsibilities

• Manage an assigned caseload and provide individualized case management.
• Coordinate behavioral health, medical, housing, benefits, education, employment, and community supports.
• Conduct outreach, engagement, and warm handoffs with clients and families.
• Develop person-centered care plans and participate in required planning processes.
• Document services, referrals, progress, and incidents in a timely manner.
• Maintain confidentiality and comply with HIPAA and program requirements.
