Job summary

La Familia is seeking a Clinical Case Manager to provide trauma-informed, clinically oriented case management to adults with serious mental health conditions transitioning from incarceration or other justice system involvement back into the community.

You will perform comprehensive assessments, develop recovery-focused plans, and deliver field-based services in homes or community settings, coordinating with partners to ensure safe reintegration and access to resources.

Qualifications

BA degree with minimum two years of experience in behavioral health, reentry services, or working with justice-involved populations, or MA in a related clinical field.
Experience working with individuals with serious mental illness, substance use disorders, and/or co-occurring conditions.
Bilingual English/Spanish highly desirable.

Responsibilities

Conduct comprehensive clinical and reentry-focused assessments.
Perform mental status examinations and ongoing risk assessments.
Develop and implement individualized recovery-oriented service plans.
Provide intensive, field-based case management during critical transition periods.
