Job summary

WestCare seeks a Family Tree Program Case Manager to deliver comprehensive case management for children, youth and families. You will coordinate behavioral health, medical, educational and social supports in a culturally responsive framework, guided by recovery-oriented and trauma-informed care principles.

As part of WestCare, you will collaborate with multidisciplinary teams, advocate for participant needs, and promote family resilience.

Qualifications

• Bachelor's degree in social work, psychology, counseling or related field plus six months of professional experience in chemical addiction or mental health counseling.
• Experience working with children, adolescents, and families preferred.
• Experience in behavioral health, substance use treatment, child welfare, healthcare, education, or community-based programs preferred.
• Familiarity with SAMHSA-funded programs and trauma-informed care.

Responsibilities

• Conduct comprehensive assessments of participant and family needs.
• Develop individualized service plans with participants and families.
• Coordinate behavioral health, primary care, educational, housing, and social supports.
• Monitor progress and revise plans; ensure continuity of care across providers.
• Engage families in planning and service delivery; provide psychoeducation and interventions.
• Maintain confidential records and document contacts per policies.
