Job summary

Shared Wellness Center in Warrington, PA, is seeking a Wellness Direct Support Professional to provide person-centered care for adults with intellectual and developmental disabilities in center and community settings.

You will support a small group, facilitate wellness activities, and help individuals gain skills, independence, and social connection while prioritizing health, safety, and rights. The role requires initiative, sound judgment, and the ability to work independently.

Qualifications

• Provide daily care, support, training, and supervision to assigned individuals.
• Understand Individual Support Plans and required supervision.
• Show strong communication, documentation, decision making, and problem solving skills.
• Ability to read and implement plans and health and safety instructions.

Responsibilities

• Provide daily care, support, training, and supervision to assigned individuals.
• Implement services and supports identified in each Individual Support Plan.
• Promote independence, community inclusion, and personal growth.
• Build respectful relationships and maintain professional boundaries.
• Provide personal care and mobility assistance as required while supervising in facility/community settings.
• Complete timely, accurate service documentation and progress notes.
• Communicate relevant information to the Wellness Program Specialist and team.
• Report incidents and health concerns with confidentiality.
