OneWell Health Care of Pennsylvania is seeking a dedicated Caregiver / Direct Support Professional in New Kensington to support individuals with IDD in home and community settings.

You will deliver hands-on daily living assistance, health and safety monitoring, and participate in team planning while complying with HIPAA and program standards. Flexible schedule and community-based work are offered.

Qualificações

• High school diploma or equivalent required.
• Experience in caregiving, home health, or direct support for IDD preferred.
• Ability to use EVV systems and document care accurately.

Responsabilidades

• Provide hands-on, person-centered support per care plan.
• Assist with personal care, daily living activities, and transportation as needed.
• Support meal preparation, nutrition, mobility, and medication administration as authorized.
• Implement behavior support strategies and monitor health and safety.
• Maintain accurate documentation and participate in trainings and team meetings.
