Shared Wellness is seeking a Direct Support Professional (CNA) in Warrington Township, PA to support individuals with intellectual disabilities in daily living, personal care, and community engagement. The role emphasizes independence, inclusion, and compassionate, side‑by‑side assistance.

You will document progress, coordinate with the Program Specialist, and ensure safety while promoting skill development and community integration. A valid driver’s license and ability to lift are required.

Qualifications

• High School Diploma or GED required.
• Valid driver’s license often required for community activities.
• Ability to lift or assist individuals with physical support.
• Strong communication, patience, empathy, and problem-solving skills.

Responsibilities

• Provide direct assistance with daily living activities, personal care, and household tasks.
• Support individuals in building independence and confidence in everyday tasks.
• Document progress and observations to support individualized plans.
• Assist with transportation and accompany to community activities and appointments.
• Maintain health and safety through standard safety protocols and de‑escalation when needed.
• Log hours in real-time and complete shift notes after each shift.
