Job summary

OneWell Health Care seeks a Dedicated Caregiver/DSP to support individuals with IDD in home and community settings in Pennsylvania. You’ll assist with daily living, mobility, meals, and medications per care plans, under supervision of the interdisciplinary team.

Qualifications

• High school diploma or equivalent required or acceptable.
• Ability to use EVV systems and electronic documentation.
• Proficient in basic computer skills and written care plans.

Responsibilities

• Provide hands-on, person-centered support to individuals with IDD in home and community settings.
• Assist with daily living activities, meal preparation, mobility, and medication help as authorized.
• Implement behavior support strategies and monitor health and safety.
• Maintain documentation, participate in trainings and team meetings.
