Job summary

Highmark Health seeks a Wellness Coach to manage a panel of enrollees with diverse health needs. You will design personalized care plans, drive behavior change, and coordinate referrals across the wellness program. Telephonic and digital outreach are used to engage members and monitor progress.

The role requires a Master’s degree or equivalent in health fields, 3 years of related experience, and the ability to work with a broad population in a health plan setting. Travel to sites is possible.

Qualifications

• Master's degree in nutrition or related field or equivalent professional experience.
• 3 years as RD/Exercise Physiologist/Wellness coach in a wellness-focused environment.

Responsibilities

• Maintain knowledge of evidence-based interventions for lifestyle coaching.
• Assess health management needs and develop care plans.
• Coordinate with multidisciplinary team to address member needs.
• Document activities in compliance with regulatory standards.
