Job summary

Highmark Inc. seeks a wellness professional to oversee a member panel, assess health needs, and coordinate care using data and clinical judgment. You will develop personalized care plans, support behavior change, and collaborate with a multidisciplinary team to improve health outcomes.

Requirements include a Master’s degree in a relevant field and at least 3 years of wellness coaching experience within a health plan or disease management setting. Travel may be required.

Qualifications

• Master's Degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology, Health or Clinical related field OR relevant experience and/or education as determined by the company in lieu of master’s degree
• Minimum 3 years as a Registered Dietitian, Exercise Physiologist, or Wellness/Lifestyle Coach in wellness-focused environments

Responsibilities

• Provide lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity, and maternal health
• Oversee a specified panel of members, assess health needs and refer to appropriate resources
• Create care plans with goals, track progress, and coordinate care with multidisciplinary teams
• Demonstrate clinical outcomes and cost savings from wellness interventions
• Document activities complying with requirements and standards
• Participate in workgroups to improve efficiency and program quality
• Undertake other duties as assigned
