Highmark Health in the United States is seeking a Wellness Coach to oversee a panel of members, assess health management needs, and design personalized care plans in collaboration with a multidisciplinary team to improve health outcomes.

The role engages with members by phone or digital channels, uses data analytics to monitor progress, ensures regulatory compliance, and contributes to program development to expand wellness services.

Qualifications

• Master’s degree or equivalent in health field as required.
• At least 3 years in a wellness/health management setting.
• Experience delivering lifestyle interventions and care planning.

Responsabilités

• Maintain knowledge of evidence-based interventions and industry standards to deliver high quality lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health.
• Maintain oversight over specified panel of members by ongoing assessment of health management needs and triaging to resources.
• Create care plans addressing identified needs, remove barriers, identify resources, and set goals with regular re-assessments.
• Demonstrate clinical outcomes and cost savings based on wellness interventions for all age groups and regions.
• Ensure activities are documented and compliant with applicable requirements and accreditation standards.
• Participate in workgroups to improve efficiency, effectiveness, and member-centric wellness services.
