Highmark Inc. seeks a Wellness Coach to oversee a panel of members with varying health needs.

Responsible for assessing health management needs, developing personalized care plans, and coordinating with a multidisciplinary team to improve outcomes. You will engage with members telephonically, digitally, or virtually, applying evidence-based interventions across tobacco cessation, weight management, sleep, stress, nutrition, and physical activity while documenting activities and ensuring

Qualifications

• Master’s degree in Nutrition, Health Education, Health Promotion, Exercise Science, Public Health, Social Work/Psychology or related field.
• 3 years as a Registered Dietitian or Exercise Physiologist or Wellness/Lifestyle Coach in a wellness-focused environment.
• Experience delivering wellness programs and disease management services.

Responsibilities

• Deliver lifestyle coaching across tobacco cessation, weight management, sleep, stress, nutrition, and physical activity.
• Create care plans addressing identified needs and coordinate with internal/external resources.
• Engage members via phone, digital, or virtual channels to improve health outcomes.
• Document activities and ensure compliance with regulatory standards.
