Job summary

Highmark Health is seeking a Wellness/Lifestyle Coach to oversee a panel of members with varying health needs. The role involves telephonic and digital engagement to drive personalized care plans, behavioral changes, and coordination of services to improve health outcomes.

The candidate will use clinical judgment with a multidisciplinary team, document all activities, and contribute to program development and efficiency.

Qualifications

• Master's degree in a health-related field or equivalent experience as determined by the company
• 3 years as a Registered Dietician, Exercise Physiologist, or Wellness/Lifestyle Coach in a wellness-focused environment

Responsibilities

• Deliver evidence-based lifestyle coaching in tobacco cessation, weight management, sleep hygiene, stress management, nutrition, physical activity and maternal health
• Oversee an assigned member panel and assess health management needs to identify clinical interventions
• Create care plans addressing identified needs, remove barriers and coordinate resources
• Demonstrate clinical outcomes and cost savings from wellness interventions
• Ensure activities are documented in compliance with regulatory standards
• Participate in workgroups to improve wellness coaching efficiency and patient-centric focus
• Perform other duties as assigned
