Job summary

Reserve Care (formerly Hospice of the Western Reserve) is seeking an on-site Weekend Telephone Triage LPN to support terminally ill patients. The role covers Saturdays and Sundays, 8:00 AM–8:30 PM, at our Cleveland headquarters.

You will coordinate care plans with the transdisciplinary team and document in the EMR while delivering compassionate, expert guidance. The ideal candidate has an active Ohio LPN license, 2–4 years of triage or clinical call experience, and strong communication skills.

Qualifications

• High School Diploma or equivalent.
• Graduate from an accredited school of professional nursing; Ohio LPN license required.
• 2-4 years of clinical support or triage/call experience in medical settings.
• Prior hospice/palliative care/home health experience preferred.
• Strong verbal and written communication skills; ability to multitask.

Responsibilities

• Answer escalated calls from families, hospital staff, and providers to assess care needs.
• Triage inbound calls for healthcare professionals and families and provide intervention.
• Document patient information accurately in EMR and systems.
• Provide level-2 medical advice to supporting agents in a ready status.
• Perform other duties as assigned.
