Job summary

Reserve Care in Ohio is seeking a licensed practical nurse (LPN) to join our Care Navigation team. The role supports referral intake, care coordination and initial clinical triage to ensure timely access to hospice services.

The ideal candidate will have an active Ohio license, hospice experience preferred, and excellent communication skills to assist patients and families through difficult times with compassion.

Qualifications

• LPN in Ohio with active license.
• Hospice experience strongly preferred.
• Excellent verbal and written communication skills.

Responsibilities

• Retrieve, monitor, and process referrals for hospice services from multiple sources.
• Learn and utilize dispatch process to schedule appointments.
• Follow up on referrals and perform direct admits to inpatient centers as needed.
• Take clinical calls from patients and caregivers and escalate appropriately.
• Communicate with primary care teams about clinical issues.
