Job summary

Reserve Care is seeking an On-Call Registered Nurse or Licensed Practical Nurse to provide after-hours, in-home hospice care on Saturdays and Sundays. The role emphasizes compassionate patient support, family education, and timely documentation while collaborating with the physician team.

Qualified candidates are licensed in Ohio, with a nursing degree and hospice experience preferred. Strong communication and independence are essential for success in this after-hours calling schedule.

Qualifications

• Licensed RN in Ohio with current credentials.
• Bachelor's degree preferred; hospice or palliative care experience is a plus.
• Strong communication skills, written and verbal, and ability to work with a team.

Responsibilities

• Assess patient and family needs (RN only).
• Provide intermittent skilled nursing per physician's direction.
• Monitor and communicate pain and symptoms to patient and family.
• Coordinate with physician(s) and team; maintain documentation.
• Provide care at patient homes and after-hours coverage.
• Attend team meetings and trainings as required.
