Job summary

Reserve Care in Cleveland, OH seeks an experienced Registered Nurse for onsite telephone triage on a part-time schedule (40 hrs/wk). The role supports terminally ill patients by assessing needs, guiding care, and coordinating with the care team using the Nursing Process.

The ideal candidate has RN licensure in Ohio and 2–4 years of triage or call-center experience, with exceptional communication and empathy. A supportive, values-driven environment awaits.

Qualifications

• RN license in Ohio is required.
• 2-4 years of clinical triage or call experience in a medical setting.
• Experience in hospice, palliative care, home health, or emergency care is a plus.
• Excellent verbal and written communication skills.
• Strong clinical judgment and decision-making abilities.

Responsibilities

• Answer escalated calls from families, hospitals, nurses and providers to assess care needs.
• Triage inbound calls for multiple healthcare professionals and families and provide intervention.
• Demonstrate calm, compassionate, and attentive care.
• Relay updated provider contact information to staff and management.
• Document patient information in the EMR and systems.
• Provide next-level medical advice to support agents in Five9.
• Perform other duties as assigned.
