Reserve Care in Cleveland, OH is seeking a Weekend Telephone Triage LPN for on-site work. The role supports terminally ill patients by assessing needs, providing medical guidance, and coordinating care with the transdisciplinary team. Weekend schedule is Saturday and Sunday, 8:00 AM-8:30 PM at our headquarters in Cleveland.

Qualifications

• High School Diploma or GED required.
• Ohio RN license or equivalent nursing credential is required.
• Excellent verbal and written communication skills.

Responsibilities

• Answer escalated calls to assess patient care needs.
• Triage inbound calls for healthcare professionals and families.
• Document patient information accurately in EMR and systems.
• Provide next-level medical advice to supporting agents.
• Perform other duties as assigned.
