Job summary

• Minimum 3 years in acute care, medical-surgical, oncology, or geriatric nursing.

Responsibilities

• Triage calls from patients and caregivers after hours.
• Perform home health visits and contribute to the care plan.
• Communicate changes in patient condition to hospice/home health team.
• Administer and evaluate medications within LPN scope.
