Job summary

Reserve Care, formerly Hospice of the Western Reserve, is seeking an on-site Registered Nurse for telephone triage. The part-time role supports families, providers and patients by assessing needs, advising care and coordinating triage in a calm, compassionate manner.

You will document interactions in the EMR, collaborate with a transdisciplinary team, and assist in coordinating the Plan of Care. Ohio RN licensure and clinical triage experience are required.

Qualifications

• Graduate from an accredited nursing program with an Ohio RN license.
• 2–4 years of clinical triage or call experience in medical settings.
• 3+ years relevant hospice, palliative care, home health, or emergency room exposure.
• Excellent verbal and written communication skills.
• Ability to multitask and manage time effectively.

Responsibilities

• Answer escalated calls from families, hospital staff, nurses’ pharmacies, patients and providers to assess care needs and advise accordingly.
• Triage inbound calls for multiple health care professionals and families and provide intervention.
• Document patient information accurately in the EMR and related systems.
• Provide advanced medical guidance to supporting agents via Five9 in real-time.
• Relay updated provider contact information to staff and management.
• Perform other related duties as assigned.
