Job summary

Centurion Health is seeking a Part-time Weekend Day Shift Discharge Planner for the SUD team at North Branch Correctional Institution in Cumberland, MD.

The role coordinates patient releases from the substance use program upon discharge or transfer to another facility and builds relationships with referral sources and community agencies serving individuals with substance abuse and co-occurring mental health disorders.

Qualifications

• Bachelor’s Degree in Psychology, Social Work, Counseling, or related field from an accredited institution; Master’s Degree preferred.
• Minimum of two (2) years of experience in release planning preferred.
• Clear written and verbal communication skills.
• Must be appropriately and actively certified in CPR.
• Corrections, forensic or criminal justice experience preferred.
• Ability to obtain a security clearance, including drug screen and criminal background check.

Responsibilities

• Coordinate a patient’s release from the substance use program upon discharge or transfer to another facility.
• Create and maintain relationships with referral sources and community agencies providing services to individuals with substance abuse and co-occurring mental health disorders.
• Identify the need to return patients to the referral source at the time of discharge.
