Job summary

Centurion Health in Baltimore, MD is seeking a Full-Time RN Discharge Planner to join the Baltimore Central Booking & Intake Center.

The RN Discharge Planner coordinates patient discharge planning from a correctional setting, under the Director of Nursing, collaborating with a multidisciplinary team to develop and implement safe, appropriate discharge plans that promote continuity of care after release. Evening shift available.

Qualifications

• Current Registered Nurse license in Maryland or ability to obtain.
• Active BLS and CPR certifications required.
• Experience as a Discharge Planner or Case Manager preferred.
• Corrections or acute setting experience is a plus.

Responsibilities

• Coordinate discharge planning for patients from a correctional setting.
• Collaborate with multidisciplinary team and Director of Nursing.
• Develop and implement safe, appropriate discharge plans to promote continuity of care after release.
