Job summary

Centurion Health is seeking a Part-time Weekend Day Shift Discharge Planner for the SUD team at North Branch Correctional Institution in Cumberland, MD.

The role coordinates patient releases from the substance use program upon discharge or transfer to another facility and builds relationships with referral sources and community agencies serving individuals with substance abuse and co-occurring mental health disorders.

Qualifications

• Bachelor’s Degree in Psychology, Social Work, Counseling, or related field from an accredited institution; Master’s Degree preferred.
• Minimum of two (2) years of experience in release planning preferred.
• Clear written and verbal communication skills.
• Must be appropriately and actively certified in CPR.
• Corrections, forensic or criminal justice experience preferred.
• Ability to obtain a security clearance, including drug screen and criminal background check.

Overview

Centurion is proud to be the provider of comprehensive services to the Maryland Department of Public Safety and Correctional Services. We are currently seeking a Part-time Weekend Day Shift Discharge Planner to join our Substance Use Disorder (SUD) team at North Branch Correctional Institution located in Cumberland, Maryland. The SUD Program Discharge Planner is responsible for coordinating a patient’s release from the substance use program upon release or leave to another facility. This role involves creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as identifying the need to return patients to the referral source at the time of discharge.

Position details: Part-Time, Weekend Day shift (16 hours per week). Location: Cumberland, MD. Pay range: $35.20 - $41.15 per hour, depending on years of experience.
