Centurion Health is seeking a Part-time Evening Shift Discharge Planner for the SUD team at Eastern Correctional Institution in Westover, MD. The role coordinates patient releases from the substance use program and builds connections with community agencies and referral sources.

The position requires a Bachelor's Degree (Master's preferred), CPR certification, and 2+ years in discharge planning or related settings, with a focus on secure and thorough care transitions.

Qualifications

• Bachelor’s Degree in Psychology, Social Work, Counseling, or related field from an accredited institution; Master’s Degree preferred.
• Minimum of two (2) years of experience working in release planning preferred.
• Clear written and verbal communication skills.
• Must be appropriately and actively certified in Cardio-Pulmonary Resuscitation (CPR).
• Corrections, forensic or criminal justice experience preferred.
• Ability to obtain a security clearance, to include drug screen and criminal background check.

Responsibilities

We are currently seeking a Part-time Evening Shift Discharge Planner to join our Substance Use Disorder (SUD) team at Eastern Correctional Institution located in Westover, Maryland.

The SUD Program Discharge Planner is responsible for coordinating a patient’s release from the substance use program upon release or leave to another facility. This person is responsible for creating and maintaining relationships with referral sources and community agencies providing services to persons with substance abuse and co-occurring mental health disorders, as well as, identifying the need to return patients to referral source at the time of discharge.

Shift: Evening 16 hours per week
