Job summary

Ambercare Hospice in Silver City NM is seeking a compassionate Registered Nurse On Call to join our newly restructured team under new leadership. The role focuses on providing high-quality in-home hospice care to patients and their families during life’s most critical moments.

Qualifications

• Graduate from an accredited nursing program with RN licensure.
• CPR and First Aid certifications are required.
• Experience in hospice or home health is preferred.

Responsibilities

• Assume primary responsibility for patient and family caseload and nursing process phases.
• Assess data from patient factors and coordinate with interdisciplinary team.
• Communicate with attending physician and agencies to coordinate care.
• Provide regular visits and reassess patient needs and care plan.
• Perform clinical pronouncement of death and support family.
• Maintain accurate records, progress notes, and care plans timely.
• Supervise and document delivery of care by Home Health Aide per guidelines.
