Job summary

Ambercare Hospice is seeking a compassionate Registered Nurse - On Call to join our newly restructured team in Silver City, NM. This role supports a culture of compassion, flexibility, and transparency, providing high-quality hospice care during life’s critical moments.

As part of the Addus HomeCare family, you will manage patient and family caseload, coordinate with the interdisciplinary team, and ensure timely documentation.

Qualifications

• Graduate from an accredited nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid certification.
• Must be knowledgeable of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance required.

Responsibilities

• Manage patient and family caseload including assessment, eligibility, admission processing, planning, implementation and evaluation of the nursing process.
• Gather data from patient and family across physical, psychological, social, and spiritual factors to guide care.
• Coordinate with attending physician and interdisciplinary team for optimal care.
• Provide regular visits to reassess needs and update the plan of care.
• Perform clinical pronouncement of death and support the family.
• Maintain accurate patient records and care plans.
• Supervise delivery of care by Home Health Aide per guidelines.
