Job summary

21st Century Home Health Services is seeking a Part-time Registered Nurse Case Manager for the Marin area. The role requires current CA RN license, BLS/CPR, and a valid CA driver’s license.

You will conduct SOC admissions, perform follow-ups, and visit patients in their homes to assess needs and deliver high-quality care. Ideal candidates have SOC/OASIS experience, strong clinical knowledge in home health, and ability to collaborate with physicians and the care team.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience with Start of Care (SOC) - OASIS is highly desired.
• Ability to work independently and with an interdisciplinary team.

Responsibilities

• Conduct SOC admissions and OASIS assessments.
• Perform follow-up and evaluation visits in assigned territory.
• Make home visits to patients within territory.
• Reconcile medications and provide wound care as directed.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and staff to ensure quality care.
• Document observations and interventions accurately.
• Initiate IV medication infusions following protocol.
