Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time RN Case Manager for the Petaluma area. You will perform Start of Care admissions, conduct follow-up visits, and visit patients at home within a defined territory.

This role includes medication reconciliation, wound care, and collaboration with physicians and the clinical team. The ideal candidate holds a CA RN license, BLS/CPR certification, and a CA driver's license, with SOC/OASIS experience being highly desirable.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting SOC admissions and OASIS is highly desired.
• High level of clinical knowledge in home health care.
• Experience with IV medication infusions is preferred.
• Self-directed, empathetic, and organized; able to work in territory-based care.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Follow up and evaluation visits in designated territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor patient conditions and report changes to clinical team.
• Collaborate with physicians and staff to deliver care.
• Document observations and evaluations of patient care.
• Initiate and administer IV medications safely per protocol.
