Job summary

21st Century Home Health Services (21HHS) is seeking a Part time Registered Nurse Case Manager to cover the Petaluma area. The role involves SOC/OASIS, home visits, and IV infusions, with a schedule that includes full weekend days or an extended 3-weekday option.

You will work with an interdisciplinary team to deliver high-quality home health care, ensuring patient safety and compassionate service while navigating a dynamic field environment.

Qualifications

• CA RN license unrestricted and in good standing.
• Current BLS/CPR certification.
• Valid CA driver's license and clean driving record.
• Experience conducting SOC - OASIS is highly desired.
• Clinical home health knowledge and high acuity patient experience preferred.

Responsibilities

• Conduct Start of Care (SOC) admissions and OASIS.
• Perform follow-ups and evaluation visits in designated territory.
• Make home visits to patients in the Petaluma area.
• Reconcile medications and provide wound care, treatments, and procedures.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and staff to deliver quality care.
• Document observations and interventions clearly and accurately.
• Initiate and administer IV antibiotics or diuretic infusions per protocol.

Part time Registered Nurse Case Manager

Anticipated Territory : Petaluma Area

Schedule: Must be available Full Days Every Saturday and Sunday (4 SOCs/day)

Also willing to consider Full Time: Saturday and Sunday plus 3 week days.

What you’ll do:

Utilize your outstanding clinical skills as you complete the initial assessment of patients to determine home care needs including:

• Conduct Start of Care (SOC) Admissions - OASIS
• Conduct follow ups, evaluation visits as needed in designated territory
• Make home visits to patients in designated territory
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures
• Monitor clients’ conditions, reporting changes to Clinical team
• Collaborate with physicians and agency staff to deliver the best care possible
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.

To be a success:

• Current and unrestricted CA RN license
• Current BLS/CPR Certification
• Valid CA Driver's License
• Experience conducting assessments and Start of Care (SOC) - OASIS is highly desired
• High level of clinical knowledge in home health care is highly preferred
• Experience with high acuity patients is a plus
• Experience working with an interdisciplinary team is highly desired
• Case Management fundamentals is highly desired
• Previous experience with IV Medication infusions highly preferred
