Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager for the Healdsburg area. You will conduct SOC admissions, perform follow-ups, and visit patients in their homes, administering IV medications and coordinating with the clinical team.

You will document care, monitor conditions, and collaborate with physicians to ensure high-quality outcomes. This role requires CA RN license, BLS/CPR, and experience in home health care.

Qualifications

• CA RN license is required and active in good standing.
• BLS/CPR certification required.
• Experience with Start of Care (SOC) and OASIS is necessary.
• Home health care knowledge and experience with high-acuity patients are preferred.
• Strong collaboration with physicians and interdisciplinary teams is essential.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Perform follow-ups and evaluation visits in the assigned territory.
• Make home visits to patients.
• Administer IV medications per protocol.
• Document observations and care management actions.
• Monitor patient conditions and report changes to the clinical team.
• Collaborate with physicians and staff to deliver quality care.
