Job summary

21st Century Home Health Services (21HHS) in Northern California is seeking a part-time Registered Nurse Case Manager to support patients in the Gilroy/Morgan Hill area. You will perform Start of Care assessments, organize follow-ups, and collaborate with physicians to deliver high-quality in-home care.

Qualifications

• Current CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care is highly preferred.
• Experience with high acuity patients is a plus.
• Experience working with an interdisciplinary team is highly desired.
• Case Management fundamentals is highly desired.
• Previous experience with IV Medication infusions highly preferred.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Conduct follow ups, evaluation visits as needed in designated territory.
• Make home visits to patients in designated territory.
• Perform assigned duties, including reconciliation of medication, wound care, treatments, and procedures.
• Monitor clients’ conditions, reporting changes to Clinical team.
• Collaborate with physicians and agency staff to deliver the best care possible.
• Initiate and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols.
• Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
