Job summary

21st Century Home Health Services (21HHS) is seeking a Part-time Registered Nurse Case Manager to cover the Novato area. The role requires visiting patients in their homes, performing SOC admissions and follow-up visits, and coordinating with physicians to ensure high-quality home health care.

The ideal candidate holds a CA RN license, BLS/CPR, and a valid CA driver’s license, with experience in SOC/OASIS and high-acuity patients.

Qualifications

• Current and unrestricted CA RN license.
• Current BLS/CPR Certification.
• Valid CA Driver's License.
• Experience conducting Start of Care (SOC) - OASIS is highly desired.
• High level of clinical knowledge in home health care.
• Experience with high acuity patients.
• Experience working with an interdisciplinary team.
• Case Management fundamentals.
• IV Medication infusions experience preferred.
• Proficiency initiating intravenous (IV) therapy.

Responsibilities

• Conduct Start of Care (SOC) Admissions - OASIS.
• Perform follow ups and evaluation visits within territory.
• Make home visits to patients in designated territory.
• Reconcile medications, wound care, treatments, and procedures.
• Monitor client conditions and report changes to Clinical team.
• Collaborate with physicians and agency staff for quality care.
• Document observations and evaluations of patient care.
• Initiate and administer IV infusions safely per protocols.
