Job summary

Responsibilities
- Actively participates in multi-disciplinary rounds to review patient status, discharge plans, and resource needs.
- Educates patients and families about illness impacts and mobilizes community resources.
- Assesses readmitted patients and identifies reasons for readmission.
- Organizes and facilitates patient and family care conferences with the team.
- Documents discharge planning evaluations, ongoing assessments, and barriers to progression.
