Job summary

Willow Creek Health Care Center is seeking an RN Weekend Supervisor to coordinate resident care during weekend operations. You will serve as clinical liaison among residents, families, physicians, and the interdisciplinary team to ensure safe, high-quality care and adherence to plans of care.

This weekend leadership role requires RN licensure, strong communication, and the ability to manage care transitions, documentation, and coordination with payers and providers in a fast-paced setting.

Qualifications

• Bachelor’s Degree in Nursing or Social Work preferred.
• Active RN license required; LVN/LPN considered.
• Minimum two (2) years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• PCC experience preferred.

Responsibilities

• Communicate regularly with residents and families regarding Plans of Care and treatment protocols.
• Attend morning stand-up meetings to facilitate interdisciplinary communication.
• Monitor resident status and adjust care interventions as conditions change.
• Oversee admissions and discharge processes for smooth transitions of care.
• Complete clinical documentation accurately and timely.
• Coordinate referrals with the facility and external providers.
