Qualifications

• Bachelors degree preferred in Nursing or Social Work.
• Active RN license required; LVN/LPN considered.
• Minimum two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits.
• PCC experience preferred.

Responsibilities

• Communicate plans of care with residents and families.
• Attend and participate in morning stand-up meetings for interdisciplinary communication.
• Answer questions about care, treatment plans and condition progression.
• Monitor resident status and adjust care interventions as conditions change.
• Organize daily workflow for admissions, current residents, and discharges.
• Complete clinical documentation accurately and timely.
• Coordinate care between residents, providers, financial services, and payers.
• Oversee admissions and discharge processes to ensure smooth transitions.
• Maintain charting and required paperwork in a timely manner.
• Exercise independent judgment when interacting with physicians, payers, residents and families.
