Qualifications

• RN licensure in the state of employment required.
• Bachelor’s degree in Nursing or Social Work preferred.
• Minimum two years of clinical nursing experience preferred.
• Knowledge of Medicare, Medi-Cal, and Medicaid programs.
• PCC experience preferred.

Responsibilities

• Coordinate and oversee resident care services during weekend operations.
• Attend morning stand-up meetings to facilitate communication.
• Monitor resident status and adjust care interventions as needed.
• Complete clinical documentation accurately and timely.
• Perform utilization review to ensure appropriate, timely care.
• Coordinate admissions and discharges for smooth transitions of care.
