Qualifications

• Must hold an active RN license in the state of employment.
• Experience with regulatory standards and facility policies.
• Ability to coordinate care with interdisciplinary teams.

Responsibilities

• Coordinate weekend resident care and ensure high-quality, safe clinical care.
• Communicate Plans of Care with residents and families.
• Attend and participate in morning stand-up meetings to facilitate interdisciplinary communication.
• Monitor resident status and adjust care interventions as conditions change.
• Complete clinical documentation accurately and timely.
• Perform utilization review to ensure timely, cost-effective care.
• Coordinate referrals and admissions/discharges for smooth transitions.

Education

Bachelor's Degree in Nursing or Social Work preferred

Active Registered Nurse (RN) license required; LVN/LPN considered

Minimum two (2) years of clinical nursing experience preferred

Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits

PCC experience preferred
