Job summary

Willow Creek Health Care Center in the United States is seeking an RN Weekend Supervisor to coordinate and oversee resident care during weekend operations, ensuring high-quality, safe clinical care and timely documentation. This role serves as a clinical liaison among residents, families, physicians, and payers while adhering to regulatory standards.

You will lead and participate in morning stand-ups, manage admissions and discharges, coordinate referrals, and maintain accurate charting.

Qualifications

• Bachelor's Degree in Nursing preferred
• Active RN license required; LVN/LPN considered
• Minimum two years nursing experience preferred
• Knowledge of Medicare, Medi-Cal, and Medicaid programs and benefits
• PCC experience preferred

Responsibilities

• Coordinate weekend resident care and oversee treatment plans
• Attend morning stand-up meetings for interdisciplinary communication
• Answer questions regarding plans of care, treatment, and progression
• Monitor resident status and adjust care interventions as conditions change
• Organize daily workflow: admissions, current residents, discharges
• Ensure accurate clinical documentation in a timely manner
• Perform utilization review to ensure appropriate, timely care
• Coordinate referrals between residents, providers, and payers
• Oversee admissions and discharges for smooth transitions of care
• Maintain charting and complete necessary paperwork promptly
• Demonstrate independent judgment when interacting with physicians and residents
• Promote collaboration among interdisciplinary partners
• Demonstrate strong time management in a fast-paced environment
• Maintain professionalism and integrity in all interactions
