Job summary

Three Oaks Hospice invites a weekend on-call Registered Nurse to join our compassionate hospice team. The RN assesses, plans, and administers skilled nursing care to patients in their homes, coordinates with the interdisciplinary team, and supports families with education and comfort.

Requirements include RN license in state, CPR certification, and 2 years’ RN experience; computer familiarity is preferred; we value interpersonal skills and resilience in complex home settings.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills supporting patients and families.
• Demonstrate nursing practice skills and resilience in family contexts.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient, family, and referring agency.
• Assume responsibility for coordination of care and continuity of services.
