Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our team. The role provides skilled nursing care in patients' residences, coordinating with the interdisciplinary team, patients, families, and referring agencies to ensure seamless care.

Qualifications include an accredited nursing degree (associate or bachelor’s), active RN license, CPR certification, and at least two years of RN experience in clinical settings; hospice/home health experience is preferred.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and driver’s license.
• Two years’ RN experience in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient computer skills; Microsoft Office experience preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their place of residence.
• Coordinate care with the interdisciplinary team, patient/family and referring agency; oversee assignment of plan of care.
