Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) to join our team and deliver skilled nursing services in patient homes. You will collaborate with the interdisciplinary care team to coordinate patient needs and maintain high-quality hospice care for diverse ages.

The role requires an RN license, current CPR, and at least two years of clinical nursing experience, preferably in hospice or home health. A valid driver’s license and computer proficiency are also required.

Qualifications

• Graduate of an accredited Associate or bachelor’s degree in Nursing.
• Current state license as an RN and current state driver’s license.
• Two years’ experience as an RN in a clinical care setting; hospice or home health experience preferred.
• Current CPR certification.
• Proficient skill using a computer is required; Microsoft Office suite experience is preferred.
• Strong interpersonal skills and ability to provide effective support to the patient and family.
• Demonstrate skill in nursing practice, ability to cope with family emotional stress and tolerance of individual lifestyles.

Responsibilities

• Administer skilled nursing care for clients in their home or residence.
• Coordinate care with interdisciplinary team, patient/family and referring agency.
• Manage care coordination and ensure continuity of services.
