Job summary

Three Oaks Hospice is seeking an On-Call Registered Nurse (RN) for weekend coverage to support our hospice patients in their homes. The role focuses on delivering skilled nursing care, coordinating with the interdisciplinary team, patients, and families while ensuring compassionate, patient-centered care.

The RN will handle on-call duties, assess clinical needs, administer treatments, and collaborate with physicians to optimize care plans in home settings.

Qualifications

• Graduate of an accredited nursing program with RN licensure.
• Current CPR certification.
• Strong interpersonal and communication skills.
• Proficient with computer and basic office software.
• Ability to support patients and families under emotional stress.

Responsibilities

• Provide skilled nursing care for patients in their homes or residences.
• Coordinate care with the interdisciplinary team, patients, families and referral sources.
• Respond to on-call weekends and maintain continuity of care.
