Job summary

Three Oaks Hospice is seeking a compassionate On-Call Registered Nurse (RN) to join our dedicated hospice team serving patients in their homes within the FTW area. This role coordinates skilled nursing care, works with the interprofessional team, and supports families during critical moments.

Responsibilities include administering nursing care, coordinating care plans, and maintaining clear communication with patients, families, and referral sources.

Qualifications

• RN license required and CPR certification.
• Two years’ RN experience in clinical care; hospice or home health preferred.
• Graduate of an accredited nursing program (Associate or Bachelor’s).

Responsibilities

• Administer skilled nursing care at patients' homes in the FTW territory.
• Coordinate care with the interdisciplinary team and communicate with families.
