Job summary

Four Seasons is seeking a nurse to provide nursing coverage to patients, families and community partners via phone and in-person visits, including hospice admissions as needed.

You will practice within the licensed scope, coordinate with the care team, educate patients, document assessments, and support after-hours triage. This full-time role serves Macon County and surrounding areas in North Carolina.

Qualifications

• Must be willing and able to care for patients with infectious disease processes unless approved accommodations.
• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
• Knowledge of HIPAA privacy rules and regulations.
• Knowledge of electronic medical records systems.
• Demonstrate excellent written and oral communication skills.
• Ability to work under pressure to meet deadlines.
• Ability to take initiative and utilize innovative techniques and ingenuity.
• Ability to interact with various departments throughout the organization.
• Ability to work flexible hours, if needed.

Responsibilities

• Providers nursing coverage to patients/families/community partners via phone & in person visits, including admission to hospice services, as needed.
• Plan, initiate, deliver, and evaluate nursing acts as required by patient care.
• Teach, delegate to or supervise staff implementing the treatment regimen.
• Collaborate with other health care providers in determining the patient and family-centered plan.
• Implement the treatment and pharmaceutical regimen prescribed by authorized personnel.
• Provide teaching and counseling about the patient's health.
• Record and report the plan for care and patient response.
• Supervise, teach and evaluate those performing nursing functions.
• Maintain safe and effective nursing care.
