Job summary

Four Seasons is seeking a Weekend Support RN to provide nursing coverage by phone and in-person visits, including admissions to hospice services. You will collaborate with providers, manage after-hours calls, and coordinate patient-centered hospice care across multiple counties in the region.

Ideal candidates have a valid NC RN license, strong organizational and communication skills, and the ability to work weekends with a team-focused approach.

Qualifications

• Knowledge of HIPAA privacy rules and regulations.
• Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation.
• Demonstrate excellent written and oral communication skills.
• Demonstrate strong organizational and interpersonal skills.
• Ability to work under pressure to meet deadlines.

Responsibilities

• Provide nursing coverage to patients and families via phone and in-person visits.
• Serve as first responder for symptom-based calls after hours.
• Coordinate hospice care with case management and other providers.
• Administer nursing care and supervise staff involved in care delivery.
• Collaborate with health care teams to develop patient-centered plans.
• Maintain accurate records of care plans and patient responses.
• Ensure safety and infection control in all nursing activities.
• Support 24-hour on-call services as required.
