Job summary

Four Seasons is seeking a Hospice RN to provide nursing coverage for patients and families in Buncombe County and surrounding areas. The role includes phone and in-person visits, hospice admissions support, and collaboration with the care team.

Qualifications

• Two years of clinical experience preferred
• Valid NC RN license
• Valid Driver’s License and proof of automobile insurance

Responsibilities

• Provide nursing coverage to patients/families/community partners via phone and in-person visits, including hospice admissions
• Serve as first responder for after-hours calls from current patients
• Collaborate with healthcare providers to develop patient-centered care plans
• Assess physical and mental health and document nursing assessments
• Coordinate and participate in hospice care implementation and case management
• Maintain safe nursing care and assist with on-call services
