Responsibilities

• Provide nursing care to hospice patients and families.
• Serve as first responder for after-hours symptom calls.
• Coordinate with case manager for follow-up and care planning.
• Assist with emergency admissions and patient transport as needed.
• Assess physical and mental status; document findings.
• Supervise and educate nursing staff as required.
• Collaborate with other providers on patient-centered care plans.
