Providence at Home with Compassus seeks an experienced Registered Nurse (RN) to provide expert care in patients’ homes as part of a collaborative, interdisciplinary team. You will assess needs, develop individualized plans of care, and deliver outcome-driven interventions under physician orders.

Qualifications

• Current RN license in the State of Employment.
• Preferred 1 year of recent home health experience.
• BLS certification required.

Responsibilities

• Conduct comprehensive patient assessments in the home setting.
• Develop and implement individualized plans of care.
• Document care activities accurately and timely.
• Collaborate with patients, families and the health team.
• Provide patient and family education and support.
• Participate in staff meetings and contribute to care planning.
